Heart To Serve Home Care – Comfort, Dignity, and Compassion in Life’s Final Chapter

At Heart To Serve Home Care, we understand the challenges that come with Alzheimer’s and other forms of dementia. Families often struggle with confusion, wandering, safety concerns, and emotional stress. Our Alzheimer’s and Dementia Care services throughout Noblesville, Fishers, Zionsville, Westfield, Greenwood, North Indianapolis, and the surrounding communities are tailored to provide compassionate, structured support, allowing your loved one to remain safe, comfortable, and connected in their own home.

What Is End-of-Life Home Care?

End-of-life care, also called comfort care or palliative support, is non-medical assistance provided to individuals in the final stages of a life-limiting illness. It focuses on:

  • Alleviating discomfort
  • Supporting daily needs
  • Providing companionship
  • Allowing family members to rest and focus on their relationship with their loved one

This care is typically provided alongside hospice or palliative care services, working collaboratively with medical teams.

Our Philosophy of End-of-Life Care

We approach end-of-life care with a heart-centered philosophy:

  • Dignity: We protect the client’s comfort, privacy, and choices at all times
  • Compassion: We show up with gentleness, patience, and respect
  • Presence: We offer steady support and reassurance, even in silence
  • Relief: We ease the burden for families and caregivers
  • Honor: We view this time as sacred and treat every client with reverence

Services We Provide During End-of-Life Care

Our caregivers provide non-clinical, practical, and emotional support, including:

  • Assistance with bathing, toileting, and grooming
  • Positioning and gentle transfers to prevent bedsores
  • Skin care, mouth care, and comfort touch
  • Meal and hydration support if desired and appropriate
  • Monitoring for signs of discomfort or distress
  • Compassionate companionship during moments of fear or agitation
  • Reading aloud, soft music, spiritual support (per request)
  • Overnight presence so families are not alone
  • Respite breaks for family caregivers
  • Non-medical transportation (e.g., final outings or family visits)
  • Light housekeeping, laundry, and home care support
  • Collaboration with hospice or palliative care teams

All care is overseen by a Registered Nurse (RN) and adjusted based on the client’s physical and emotional needs.

Support for the Whole Family

We support not just the client, but the entire care circle:

  • Education on what to expect as conditions change
  • Emotional support and presence
  • Encouragement to participate in meaningful moments
  • Guidance on comfort measures and home safety
  • Space to grieve, reflect, and breathe

Grief and Bereavement Support

Our commitment doesn’t end at the bedside. After a loved one passes, families often feel overwhelmed, numb, or unsure of what to do next. We provide gentle follow-up and offer referrals to local grief counselors, support groups, and pastoral care providers in Indianapolis.

We also provide resources to help families:

  • Process emotions
  • Preserve memories
  • Navigate the next steps with clarity and support

You’re never alone — even after care ends.

What Makes Heart To Serve Different

  • RN-led plans tailored for comfort and dignity
  • Experienced caregivers trained in gentle end-of-life care
  • Spiritual and emotional support upon request
  • Emergency backup staffing and flexible schedules
  • No long-term contracts
  • Care delivered with faith, reverence, and humility

Whether it’s a few final days or several months, we are honored to serve.

Can I Use End-of-Life Care Without Hospice?

Yes. While many of our clients receive hospice support from a licensed provider, families may also use our services as a standalone comfort care when:

  • Hospice hasn’t been initiated yet
  • Additional personal care or companionship is needed
  • A faith-based, home-centered approach is preferred
  • Overnight presence is needed beyond hospice hours

We do not replace hospice but provide critical in-home support in harmony with medical teams.

How to Begin End-of-Life Home Care

Starting care is as simple as a phone call. We offer a free consultation with our RN to understand your loved one’s condition, home setup, and comfort needs. Care can often begin within 24–48 hours.
